A Harvard expert shares his Ideas on testosterone-replacement therapy

It could be stated that testosterone is the thing that makes guys, guys. It gives them their characteristic deep voices, big muscles, and facial and body hair, distinguishing them from women. It stimulates the development of the genitals , plays a role in sperm production, fuels libido, and contributes to normal erections. It also boosts the production of red blood cells, boosts mood, and aids cognition.

Over time, the testicular"machinery" that makes testosterone slowly becomes less powerful, and testosterone levels start to fall, by about 1 percent per year, starting in the 40s. As guys get in their 50s, 60s, and beyond, they might begin to have symptoms and signs of low testosterone like reduced libido and sense of energy, erectile dysfunction, diminished energy, decreased muscle mass and bone density, and nausea. Taken together, these symptoms and signs are often referred to as hypogonadism ("hypo" significance low functioning and"gonadism" referring to the testicles). Researchers estimate that the condition affects anywhere from two to six million men in the United States. Yet it's an underdiagnosed issue, with just about 5 percent of those affected undergoing therapy.

But little consensus exists about what constitutes low testosterone, when testosterone supplementation makes sense, or what dangers patients face. Much of the current debate focuses on the long-held belief that testosterone may stimulate prostate cancer.

He's developed specific expertise in treating low testosterone levels. In this interview, Dr. Morgentaler shares his views on current controversies, the treatment strategies he utilizes his patients, and why he believes specialists should rethink the potential link between testosterone-replacement therapy and prostate cancer.

Symptoms websitesthis post and have a peek here diagnosis

What signs and symptoms of low testosterone prompt the average person to find a doctor?

As a urologist, I tend to observe guys because they have sexual complaints. The primary hallmark of reduced testosterone is low sexual desire or libido, but another may be erectile dysfunction, and some other man who complains of erectile dysfunction should possess his testosterone level checked. Men can experience other symptoms, such as more trouble achieving an orgasm, less-intense orgasms, a lesser quantity of fluid from ejaculation, and a sense of numbness in the manhood when they see or experience something which would usually be arousing.

The more of the symptoms there are, the more probable it is that a man has low testosterone. Many physicians often discount those"soft symptoms" as a normal part of aging, but they're often treatable and reversible by normalizing testosterone levels.

Aren't those the very same symptoms that men have when they are treated for benign prostatic hyperplasia, or BPH?

Not exactly. There are a number of medications which may lessen sex drive, such as the BPH drugs finasteride (Proscar) and dutasteride (Avodart). Those drugs may also reduce the amount of the ejaculatory fluid, no wonder. However a decrease in orgasm intensity usually doesn't go together with treatment for BPH. Erectile dysfunction does not ordinarily go together with it either, though surely if somebody has less sex drive or less attention, it's more of a challenge to have a good erection.

How can you determine if a person is a candidate for testosterone-replacement therapy?

There are just two ways we determine whether somebody has low testosterone. One is a blood test and the other one is by characteristic symptoms and signs, and the correlation between these two methods is far from ideal. Generally guys with the lowest testosterone have the most symptoms and men with maximum testosterone have the least. But there are a number of men who have low levels of testosterone in their blood and have no signs.

Looking purely at the biochemical amounts, The Endocrine Society* considers low testosterone for a total testosterone level of less than 300 ng/dl, and I think that is a sensible guide. But no one really agrees on a number. It's similar to diabetes, in which if your fasting glucose is over a certain level, they'll say,"Okay, you've got it." With testosterone, that break point is not quite as apparent.

*Note: The Endocrine Society publishes clinical practice guidelines with recommendations for who should and should not receive testosterone therapy.

Is total testosterone the ideal thing to be measuring? Or should we be measuring something different?

This is another area of confusion and good discussion, but I do not think it's as confusing as it is apparently in the literature. When most physicians learned about testosterone in medical school, they heard about overall testosterone, or all the testosterone in the human body. However, about half of the testosterone that is circulating in the bloodstream is not readily available to cells. It's closely bound to a carrier molecule known as sex hormone--binding globulin, which we abbreviate as SHBG.

The biologically available part of total testosterone is known as free testosterone, and it is readily available to cells. Though it's just a small fraction of the total, the free testosterone level is a fairly good indicator of reduced testosterone. It is not perfect, but the correlation is greater than with testosterone.

This professional organization urges testosterone treatment for men who have both

Therapy is not Suggested for men who've

  • Breast or prostate cancer
  • a nodule on the prostate that can be felt during a DRE
  • a PSA higher than 3 ng/ml without additional evaluation
  • that a hematocrit greater than 50 percent or thick, viscous blood
  • untreated obstructive sleep apnea
  • severe lower urinary tract symptoms
  • class III or IV heart failure.

Do time of day, diet, or other factors affect testosterone levels?

For years, the recommendation has been to get a testosterone value early in the morning because levels start to fall after 10 or even 11 a.m.. However, the information behind this recommendation were attracted to healthy young men. Two recent studies demonstrated little change in blood glucose levels in men 40 and mature within the course of this day. One reported no change in typical testosterone till after 2 p.m. Between 2 and 6 p.m., it went down by 13%, a modest sum, and probably insufficient to influence identification. Most guidelines nevertheless say it is important to perform the evaluation in the morning, but for men 40 and above, it probably doesn't matter much, as long as they get their blood drawn before 6 or 5 p.m.

There are a number of rather interesting findings about dietary supplements. For example, it appears that individuals that have a diet low in protein have lower testosterone levels than males who eat more protein. But diet hasn't been studied thoroughly enough to make any clear recommendations.

In the following guide, testosterone-replacement treatment refers to the treatment of hypogonadism with exogenous testosterone -- testosterone that is manufactured outside the body. Based upon the formulation, therapy can cause skin irritation, breast enlargement and tenderness, sleep apnea, acne, reduced sperm count, increased red blood cell count, and other side effects.

In a recent prospective study, 36 hypogonadal men took a daily dose of clomiphene citrate for at least three months. Within four to six months, all of the men had heightened levels of testosteronenone reported some side effects during the year they had been followed.

Since clomiphene citrate is not accepted by the FDA for use in men, little information exists about the long-term ramifications of taking it (including the probability of developing prostate cancer) or whether it is more capable of boosting testosterone than exogenous formulas. But unlike exogenous testosterone, clomiphene citrate preserves -- and potentially enhances -- sperm production. This makes drugs such as clomiphene citrate one of only a few options for men with low testosterone who wish to father children.

Formulations

What forms of testosterone-replacement treatment are available? *

The earliest form is the injection, which we still use since it's cheap and since we faithfully get good testosterone levels in almost everybody. The disadvantage is that a man should come in every couple of weeks to find a shot. A roller-coaster effect may also occur as blood glucose levels peak and return to research.

Topical treatments help preserve a more uniform amount of blood testosterone. The first kind of topical therapy has been a patch, but it has a very high rate of skin irritation. In 1 study, as many as 40% of people that used the patch developed a red area in their skin. That restricts its use.

The most widely used testosterone preparation in the United States -- and also the one I begin almost everyone off with -- is a topical gel. There are just two brands: AndroGel and Testim. According to my experience, it tends to be consumed to great levels in about 80% to 85 percent of guys, but that leaves a significant number who don't absorb sufficient for it to have a positive impact. [For specifics on several different formulations, see table ]

Are there any downsides to using dyes? How long does it require them to get the job done?

Men who start using the implants need to return in to have their testosterone levels measured again to make certain they are absorbing the right amount. Our goal is that the mid to upper range of normal, which usually means approximately 500 to 600 ng/dl. The concentration of testosterone in blood really goes up quite fast, within a few doses. I normally measure it after two weeks, although symptoms may not alter for a month or two.
